Introductory Workshop on How To Plan Ahead

At Your Office? With Your Community Group? In Your Home?

Whether end of life comes naturally, as the result of an accident, or the final chapter of a disease, planning ahead can make things easier. It gives you the chance to consider your values about quality of life before confronting life and death decisions. It helps you understand in advance what measures you would or would not want taken depending on the severity of your illness or injury. And most importantly, it gives you the chance to die on your terms, by sharing your end-of-life wishes with medical professionals, loved ones and people who support you. 


This workshop will give you guidance on where to start planning and documenting your wishes through Advance Directives and how to have conversations with those close to you about what quality of life means to you. We will also answer questions and concerns, provide helpful planning tips and guide you to free and useful resources online.

Advice on Advance Directives

If you are incapacitated and cannot communicate for yourself, Advance Directives give guidance as to what life-sustaining treatment you do or do not want. These forms are used by medical professionals, caregivers, and most importantly, your Durable Power of Attorney for Health Care to help honor your wishes. 


We can offer guidance on how to select, complete, formalize and share Advance Directives, including your Living Will/Healthcare Directive and your Durable Power of Attorney for Health Care.

End of Life Planning with Dementia/Alzheimer's

With Alzheimer's on the rise, it is increasingly important to understand what end-of-life choices can be made in advance of the onset of the disease and eventually losing the capacity to make decisions. In addition, it's important to give direction as to what measures you do or do not want taken to extend your life as your quality of life and cognitive ability declines. 


We can walk you through the conversations and documents you'll want to have and share with those that support you as it specifically relates to dementia.
